Sick Notes

You can request sick notes via this form.

The Doctor may contact you to clarify certain details and confirm if a sick note is possible.

If you have been off work for less than 7 (calendar) days; you can complete your own sick note using this form. You DO NOT need a Doctor’s note until there have been more than 7 calendar days of absence.

If the sick note is needed for a past date; we need documentation that an illness has occurred, such as a telephone or face-to-face appointment here or with an out of hours provider.
